Supporting Your Mental Health and Personal Recovery Journey
At Adapt Family Health Solutions, we understand that living with a psychosocial disability means having “good days” and “tough days.” A psychosocial disability comes from mental health challenges that can make it hard to do everyday things, like hanging out with friends, working, or looking after your home.
Our team doesn’t just look at a diagnosis. We look at you. We focus on your strengths and help you find ways to manage your mental health so you can live the life you want in South Morang, Shepparton, or Adelaide.
What is a Recovery Coach?
If you have psychosocial support in your NDIS plan, you can work with a Recovery Coach. They are different from a standard support worker because they specialize in mental health:
- Building a Recovery Plan: We sit down and talk about what “recovery” means to you. It’s not always about being “cured”—it’s about living a full and meaningful life even with mental health challenges.
- Coordination of Care: We help you talk to your doctors, therapists, and the NDIS to make sure everyone is working together to support you.
- Motivation and Hope: On days when things feel heavy, your coach is there to help you stay focused on your goals and remind you of how far you’ve come.
Practical Support for Daily Life
Mental health challenges can make daily routines feel like a mountain. We provide hands-on help to make things easier:
- Social Connection: We help you slowly build the confidence to join community groups or reconnect with family and friends at your own pace.
- Routine Building: We help you create a daily “rhythm” that includes self-care, healthy eating, and sleep—all the things that help keep your mind steady.
- Crisis Planning: We work with you to create a plan for when you feel a “dip” in your mental health, so you and your family know exactly what to do to stay safe.
Your Voice, Your Choice, Your Journey
Recovery is not a straight line, and you are the expert of your own life. We use a “person-centered” approach, which means you are always in the driver’s seat. Our team listens without judgment and respects your choices. Whether you want to focus on finding a job, going back to study, or simply feeling safe enough to leave the house, we move at a speed that feels right for you. We are here to provide the tools, but you decide which path we take.
